MEMBERSHIP
What do I need to do to qualify for membership? Interested designers must fill out an online application at https://carpetlandonline/services/interior-design-program/interior-design-program-application or in-store at Carpet Land. Applications must be accompanied by the following supporting documents:
- Must provide a W-9 form
- Provide Sales & Use Tax Permit and ST-4 Form (if applicable)
- Must submit one of the following:
- Business License
- Certificate of Incorporation or Certificate of Organization
- Proof of graduation from a design related program

APPLY ONLINE
Membership approval is contingent upon receipt of approved credentials and a completed application. Non- designer related businesses or professionals do not qualify for the program. One card will be issued per approved business and all referral rewards checks will be written to the business name listed on the application. You must provide Carpet Land with a completed W-9 form or your membership will not be considered. Approved Interior Design Affiliates will receive an email within 10 business days confirming membership and benefits. Please allow 4 to 6 weeks for receipt of permanent membership card. This is a membership application, not a credit card application.
